Michigan’s Path to a Prosperous Future, the third in a <a href="https://crcmich.org/publications/prosperous-future" target="_blank" rel="noopener">series of five reports</a> by the Citizens Research Council and Altarum looking at Michigan‘s future prosperity, has revealed some startling insights about Michigan&#8217;s health trends. <a href="https://crcmich.org/PUBLICAT/2020s/2023/prosperous-future-health.pdf" target="_blank" rel="noopener">According to the report</a>, Michiganders are, on average, less healthy than other states in many categories such as life expectancy, self-reported health status, and mental health.  
These declining health rates become more staggering considering the wide disparities between races, socioeconomic status, and where someone lives, adding that life expectancy can vary as much as 29 years from one neighborhood to another. Infant mortality rates are another widely disproportionate statistic, with Black infant mortality rates being 2.7 times higher than White rates.

These issues in healthcare may also be impacted by Michigan’s aging population and the <a href="https://wordpress.detroitpbs.org/business/millennials-make-up-a-majority-of-the-global-workforce-can-michigan-attract-them-to-move-here/" target="_blank" rel="noopener">decline of young people</a> moving to or staying in the state, the CRC report states. The lack of youth may also add to the growing health problems due to a projected <a href="https://wordpress.detroitpbs.org/caregiving/young-caregivers-need-more-support-theres-an-app-for-that/" target="_blank" rel="noopener">scarcity of caregivers</a>. Michigan did rank favorably, compared to the national average, in the traditional healthcare sector, showing much of the state is insured, has lower-than-average healthcare costs, and boasts a higher-than-average physician count, though resources are not equally dispersed across the state. 
Director of the Department of Health, Human, and Veterans Services at Wayne County, and former gubernatorial candidate, <a href="http://abdulelsayed.com/" target="_blank" rel="noopener">Dr. Abdul El-Sayed</a> joins One Detroit producer <a href="https://twitter.com/WillofMichigan" target="_blank" rel="noopener">Will Glover</a> to talk about the report&#8217;s findings and what it means for residents across the state. Dr. El-Sayed shares his concerns about where Michigan’s healthcare is trending and explores how public policy and the environment one lives in can also contribute to poor health.

Will Glover, Producer, One Detroit: Is it true to say that Michigan&#8217;s healthcare outcomes have been sliding over the past couple of decades?
Dr. Abdul El-Sayed, Director, Wayne County Department of Health, Human, and Veteran Services: Well, I really appreciate the question because I think it allows us to think a little bit about some of the context. We, as a state, are struggling to attract and keep people here. And it turns out that people want to go to places where they can live their healthiest and happiest lives. And one of the challenges that we sometimes have in Michigan is that we haven&#8217;t necessarily built a lot of the infrastructure that we need for people to be able to do that thing.
And that&#8217;s not just hospitals. I mean, I want you to think about when you go to a hospital. You don&#8217;t go to a hospital or a clinic when you&#8217;re feeling well. You go there after you get sick in the first place. And so it&#8217;s not about just providing the means of people to get healthy after they&#8217;ve been sick. It&#8217;s about keeping them healthy in the first place.
Part of the challenge that we have in Michigan is, unfortunately, we have not necessarily built what we need to be able to do that, whether it&#8217;s the kind of density of healthy foods or a walkable environment, or even clean air. And I want to be specific here. For a lot of Michiganders, those things are readily available, but for too many Michiganders, they&#8217;re not. And too often those things are patterned by race and by income.
And so you end up having pockets of our state where too often people are breathing dirty air, they have questionable access to clean water, and they can&#8217;t necessarily get around. We think about the brutal interplay between an unwalkable community and then the fact that insurance rates for cars are so high and that puts you in a position where you have to ask, how do I even get to work?
Will Glover: In Michigan, the Citizens Research Council (CRC) report highlights that the life expectancy variation from one neighborhood to another can be as much as 29 years. That, to me, is a very staggering number. How is that variation possible?

Dr. Abdul El-Sayed: That kind of thing. That injustice of having a longer access to a healthier life ought to be something that all of us step back and really, really ponder upon. It is about all of the circumstances in a daily life that benefit health in one community and really can take it away in another.
It starts with the simple air we breathe. One project we just kicked off here at Wayne County is about building an air monitoring network simply because we know that while all of us have had to think a little bit more about our air over the past several weeks, given the Canadian wildfires, there are a lot of communities, particularly in southwest Detroit, places like River Rouge or South Dearborn, where they&#8217;ve got to think about air quality every single day. And if the very air you breathe that you&#8217;re extracting oxygen from also brings with it pollutants that are going to damage your lungs and damage your heart and damage your kidneys, not just for adults and seniors, but for children as they are building their body in the first place, you got to imagine how that hobbles folk.
Then you think about the access to a walkable environment. We know what we call neat or non-exercise activity is one of the most important predictors of overall physical health. It&#8217;s not just going for a run or exercising for half an hour, an hour, every day, or every other day. It&#8217;s also just how much activity you get in your daily life as a function of the life that you live.
Now in Michigan, obviously, we&#8217;re a community that was built by the automotive industry and so everything was funneled in that direction. So rather than being able to walk to the bus stop, get on a bus, walk from the bus stop to work every day, and do that sustainably, unfortunately, our public transit systems just haven&#8217;t been invested in in the way needed to be able to allow for public transit and all the activity that comes with that.
Will Glover: If we change our ways, if we start to address the issues that we need to address, what could our future look like?
Dr. Abdul El-Sayed: One of the most important things that really does benefit public health, considering the fact that right now one of the major killers is mental illness, whether that comes out in suicide or that comes out in substance use. One of the most important features of daily life that matter to us is one another and the ability to look one another in the eye and have a real-life conversation where we can read and feel one another&#8217;s emotions. And a healthier society, a place where we&#8217;ve invested in public spaces, for example, that make coming together easier and more enjoyable is a more joyful life. And health is joyful.
Being healthy and being able to live to the full extent of your capacities is a joyful thing. And so the lived experience of being healthier collectively is that we have one another more and that we have more of ourselves to be able to enjoy one another and to be able to enjoy the lives that we lead. And I think that&#8217;s what&#8217;s at stake. And so I just want folks to appreciate that these- when we talk about health statistics, it&#8217;s easy to just think about a bunch of numbers. And people don&#8217;t really quite care what the number of life expectancy is, but they do care about what the experience of living in a society where more of us are healthy ought to be.
And I think that just becomes a more joyful, more collectively oriented and engaged society where we spend more time with one another and the people that we love and we&#8217;re able to take advantage of those 29 years of life that some people don&#8217;t get to lead right now, leading that together. And I think that&#8217;s what life is about&#8230; Is spending meaningful time with the people that you love. And when some people lose 29 of their years and you think about the pockets where that&#8217;s concentrated, we are literally robbing ourselves not just of health, but the joy that living can offer. And that is a question of public policy.
And I know a lot of folks think, well, health is about what I choose to do. It&#8217;s about the food that I eat or the exercise that I choose to do. Yeah, those are all true. And no doctor is ever going to tell you don&#8217;t eat healthy and don&#8217;t exercise. But the choice to do those things doesn&#8217;t actually exist evenly for everybody.

Michigan’s health declines: Below-average outcomes, disparities and funding gaps pose challenges for the state

Melody Williams, a legally blind paratransit rider in Detroit, wakes up every morning at 3 a.m. to get ready for her 4:30 a.m. ride to the dialysis center for her appointment. While the paratransit services eventually get Williams to her location, <a href="https://www.bridgedetroit.com/city-transit-service-in-question-for-riders-with-disabilities/">it&#8217;s not often without a bump in the road</a> she says. Under the city’s former paratransit services, provided by Transdev, riders might be dropped off at the wrong location, picked up late, or not treated with care.  
Williams wasn’t alone in her criticism of MetroLift, the Detroit Department of Transportation’s paratransit service, either. The paratransit services faced enough scrutiny that in November 2022 Detroit City Council members <a href="https://www.bridgedetroit.com/transit-detroiters-with-disabilities-at-risk/">voted against a contract extension for Transdev</a>, and in December <a href="https://www.bridgedetroit.com/duggan-paratransit-service/">Mayor Mike Duggan used his emergency powers</a> to ensure the city still offered paratransit services under the Detroit Department of Transportation (DDOT).

Since then, DDOT has continued to fill the paratransit services with temporary contractors as they search for a long-term provider. At the same time, the Department of Justice is looking into if the city has violated the Americans with Disabilities Act by failing riders with disabilities.  
One Detroit contributor Bryce Huffman sat down with Williams to learn more about her experiences as a rider and what she hopes to get from the new service provider. Plus, Huffman talks with Mikel Oglesby, executive director of transit for Detroit, about what the future holds for Detroit’s paratransit services.

Bryce Huffman, BridgeDetroit, Reporter: It&#8217;s half past four am on East Ferry Street. The roads are nearly empty on this early Friday morning, but Detroiter Melody Williams is up and waiting for the paratransit van to arrive. 
Melody Williams, Legally Blind Paratransit Rider: Getting up this early is just part of her weekday routine. Today, she&#8217;s using it to get to her dialysis treatment. On Mondays, I do dialysis, so I get a one-way trip. My day start at 3:00 in the morning and my ride is from 4:30 to 5:00.
Around about 10:00, I&#8217;m off the machine and then my family members come and pick me up and then I go to work.  And then in the afternoon, I get off about 2:30. So I have another ride scheduled for MetroLift. That&#8217;s on Monday. And on Tuesday from 9:00 I go to work and Wednesday back to dialysis. Thursday, work again. And then on Fridays I do dialysis. 
Bryce Huffman: The van picks up and drops off other passengers on the way to Williams treatment. Like many people using the service, Williams is visually impaired. 
Melody Williams: It&#8217;s considered as legally blind. So I have vision, but not clarity. Like if you put a Ziploc bag to your eye. That&#8217;s what I see. 
Bryce Huffman: Williams has been legally blind since 2010. She spent ten months in Kalamazoo learning to navigate life without full vision. That included learning how to catch the bus now that she couldn&#8217;t drive herself. 
Melody Williams: When I came back to Detroit, I wanted to continue that. So I wouldn&#8217;t be stuck in the house and dependent on my girlfriends or my aunts or my sons or my friends. You know, different people&#8230; my neighbor, to take me backward and forth. I signed up for MetroLift. 
Bryce Huffman: MetroLift, is Detroit&#8217;s Paratransit service. A transportation option specifically for people living with disabilities or other mobility challenges. William&#8217;s has been using the service since 2017. How do you make the decision to schedule or get a ride from someone else? 
Melody Williams: When I do dialysis on Monday, Wednesday and Friday, when I get off that machine about 10:00, I be extremely sick and weak. So therefore, if I take MetroLift, there is no telling when I might get home and how I will make it. So therefore my family members will come and pick me up on Monday, Wednesday and Friday. Everybody got a certain day. 
Bryce Huffman: In November, City Council voted against a new contract for Transdev, the company that operated the Paratransit service. Riders who use the service often complain about how unreliable the company was. Williams said drivers would leave passengers in the wrong locations, pick them up late, and sometimes not pick them up at all. 
Melody Williams: It&#8217;s sometimes frustrating. But we don&#8217;t have no other choice. How are going to get the doctors. How are we going to get to the grocery store? How are we going to get to our treatments? Cancer patients and stuff they do. They use MetroLift as well. They&#8217;ll be own on scooters. It&#8217;s horrible. 
Bryce Huffman: Detroit&#8217;s Paratransit service could look very different by summer, however. At the Detroit Department of Transportation office, we sat down with Mikel Oglesby. He&#8217;s the executive director of Transit for the City.  
At the end of last year, obviously, there was a little bit of debate about if there should be a contract extension or approval for Transdev. A lot of folks in the 88 community weren&#8217;t fans of them. Could you briefly describe just kind of how that resolved itself from when the contract was rejected to where we are now? 
Mikel Oglesby, Executive Director of Transit, City of Detroit: Sure. It&#8217;s pretty straightforward. We put out an RFP, a request for proposal for service. There were two providers that responded. One was Peoples and one was Transdev. The end result was that the 30% People&#8217;s Express were approved and the other 70% was not. 
Bryce Huffman: In December, Mayor Mike Duggan used his emergency powers to make a temporary fix to the problem. 
Mikel Oglesby: I was really excited that he gave us the tools to do our job and that was to move forward with additional providers to be able to fill that gap. 
Bryce Huffman: The city started seeking out new bids for the long-term contract last month. Now that DDOT runs the entire system&#8230; What sort of changes have you noticed as the person in charge of running the system and what have you been hearing from the people who use the Paratransit system? 
Mikel Oglesby: Well, I will tell you, we took it over the beginning of the year and the response has been like night and day. They like the customer service. We&#8217;re providing professional service. We are handling complaints expeditiously. And the overall general response is very positive. 
Bryce Huffman: But what do people who rely on the system, like Williams say about the new companies providing service, and how does she compare it to Transdev? You rode with People&#8217;s Express today. If you could rate them out of ten, what would you rate People&#8217;s Express and then what would you rate Transdev out of ten? 
Melody Williams: Okay People&#8217;s&#8230; They always have good drivers and therefore one thing I like about People&#8217;s now they have cars. So therefore a lot of seniors that cannot get up in those high vans can get in a lower car.  So I say service for now&#8230; I give them about an eight, eight and a half. 
Bryce Huffman: Okay, that&#8217;s pretty good. And what about Transdev? 
Melody Williams: Zero. 
Bryce Huffman: And as for the long-term solution, six months from now, what do you want to see happen? What would you like to see DDOT do? 
Melody Williams: I would like to be consistency. With our driver&#8217;s, consistency. A clean van. Take care of your seniors. Get out of the car and help them when you know they&#8217;re handicapped and they barely can get in. Definitely be on time. I know sometimes it&#8217;s hard to do that because when you&#8217;re working with a large city, it&#8217;s more than one client. It&#8217;s over 1,000 rides a day. 
Bryce Huffman: The Department of Justice is now looking to ensure the city isn&#8217;t violating the Americans with Disabilities Act by failing disabled passengers. 
Melody Williams: So, I understand it&#8217;s hard, but they need a better system, a much better system than what we have now.

If the scope of <a class="gnt_ar_b_a" href="https://www.democratandchronicle.com/story/news/politics/2021/10/07/biden-build-back-better-budget-proposal-could-cost-zero-changes/5990447001/" target="_blank" rel="noopener" data-t-l=":b|e|inline click|${u}">President Joe Biden&#8217;s social spending plan</a> is too large to grasp, then consider Nancy Slomba of Western New York state.
She spends 84 hours a week caring for her son Joe Slomba, who has cerebral palsy and needs assistance to bathe, get in and out of bed and leave the family’s house.
The 59-year-old resident of a small Wyoming County town about 35 miles east of Buffalo gets paid $12.50 an hour to care for Joe through a New York state-based program. She and Joe&#8217;s twin brother, Ben, split the caregiving hours throughout the week.
But they&#8217;re making less per hour than they&#8217;d make working at McDonald’s. Joe&#8217;s care needs mean they can&#8217;t get other jobs, and there aren&#8217;t home care workers available in their rural county to share some of the caregiving duties.
“I’m exhausted,” Slomba said. “You’re not going to leave your family member, yet you’re not making enough money.”
Slomba is one of millions who care for their loved ones in the United States. In 2020, 48 million people in the U.S. provided unpaid care to an older adult or individual with health or functional needs. Many live in old industrial cities in places like Upstate New York and Michigan. A significant number are persons of color.
Those who cannot care for their loved ones themselves hire home care workers — a profession that is adding more new jobs than any other field in the American economy this year, but which struggles to attract and keep workers due to a dismal track record of low wages and scant benefits.
President Joe Biden made the untangling of this nationwide dilemma a top priority of his administration, earmarking a $400 billion chunk of his initial infrastructure agenda, titled the American Jobs Plan, to tackle home and community-based care. The funding would phase in over eight years.

For Susie Young, the days before she was a unionized caregiver weren’t ones to cheer about.
“Before the union came in, we had nothing,” she said. “No training. Forget about a paid holiday or vacation. &#8230; There’s many workers in this country today that’s where they are.”
Young, 73, lives and works in Spokane, Washington, where she has been a home care worker for about 35 years. She helps a developmentally delayed man who’s high-functioning enough to work outside the home and ride the bus but still needs reminders and other assistance to keep him on track.
Her workload, a couple of days a week, is less than it once was. Decades ago, Young, who’s a founding member of the executive board of the Service Employees International Union Local 775, was caring for people suffering from HIV and was a union skeptic. Having to press her employers for basic protective gear, a situation that’s since improved, was what, she said, changed her mind about the union.

Paula Duren, a Detroit-based psychologist, is one of an increasing number of caregivers of elderly adults who felt overwhelmed by the task of caring for aging parents, both of whom suffered from dementia.
“There were moments that felt like you didn’t even know what to do,” Duren said. “It brings about a helplessness.”
Duren was a panelist at Thursday night’s virtual forum hosted by the New York &amp; Michigan Solutions Journalism Collaborative, a group of news, community and academic organization that cover chronic problems with a solutions-focused lens, and Strides for Seniors, an annual month-long event series focused on Detroit’s senior centers.
The event, presented by collaborative members Detroit Public Television and Urban Aging News, featured journalists, caregiving experts, and community members who have served as caregivers for their loved ones.
The number of unpaid family caregivers has been rising in the United States, up from 43.5 million in 2015 to 53 million in 2020, according to a recent study by the AARP Public Policy Institute. The causes are numerous — from the aging of the Baby Boomer generation to a shortage of paid family caregivers. But despite formidable challenges, panelists discussed potential solutions being attempted across the United States.
After her experience, Duren founded Universal Dementia Caregivers. The non-profit organization offers support and education to those affected by dementia-related issues through services such as family mediation, legal guidance and workshops on caretaking skills and self- management.
“One of the things we do is invite attorneys in so people can ask their questions,” Duren said. Family caregivers “never think about the fact that (they) might need a power of attorney or a will might need to be established.”
<h2>Paid caregiver, funding shortage</h2>
The strain of caregiving isn’t unique to family caregivers; professional caregivers are also struggling.
“It’s a job that many spoke … about feeling very under-appreciated,” said Sarah Rahal, a reporter for The Detroit News, during the event.
Paid caregivers’ turn over at a rate of 82 percent, and the workforce is currently in need of 34,000 more professional care workers, she said. This is a situation that is “expected to get much worse. There are thousands in Michigan who are in desperate need,” Rahal said.
One solution is giving caregivers — paid and unpaid — more money.
The recent Biden administration infrastructure proposal originally included funding for increased caregiver pay, but that provision was left out of the deal struck this summer between the White House and a bipartisan group of senators.
Billions of dollars were proposed to go to expanding access to long-term care services in the home rather than receiving care at institutions, and the second would be to make caregiving jobs worth having, said Michael Kilian, editor of the Rochester Democrat and Chronicle.
Professional caregiving jobs “don’t pay well and… are very difficult,” Kilian said.
But Biden’s proposal did not include similar funding for unpaid family caregivers. The AARP has found that unpaid caregivers pay on average more than $7,000 a year out of pocket caring for loved ones.
According to Madeleine O’Neill, a reporter with the USA Today Network, there is currently a proposal in Washington that would provide unpaid family caregivers a tax credit up to $5,000 and allow Medicaid to cover more home-based services.
This proposal, if passed, “would be transformational for family caregivers in a sense that family caregivers might actually be able to access professional care for their loved ones and return to the workforce,” O’Neill said.
<h2>Needing a break</h2>
In addition to financial stress, caregivers also cope with physical and emotional strain.
For instance, Rahal pointed to the difficulty of the work itself, stating that caregivers are injured at a higher rate than even truck drivers in the United States.
Duren, the Detroit psychologist, says caregivers will neglect self-care when they put their loved ones first.
The Hannan Center, a senior center in Detroit, offers respite care in the community through a program called Daybreak. This respite care service allows caregivers a break at affordable rates, sometimes as low as $5 per hour.
“Sometimes it’s just a few hours, sometimes it could be five days a week,” said Vincent Tilford, the center’s executive director. “It helps to improve that relationship between the care partner and the person that they are caring for.”

A live virtual public forum explores strategies for caregivers and the community to provide the support our seniors deserve, presented by the New York &amp; Michigan Solutions Journalism Collaborative

Whether they are paid workers or family members, people who provide care for older adults face numerous challenges, from physical burdens and economic hardships to loneliness and depression. 
More than one in five Americans act as a family caregiver, according to a <a href="https://www.aarp.org/caregiving/basics/info-2020/unpaid-family-caregivers-report.html">2020 report</a> from the AARP Public Policy Institute, many of whom receive no compensation and must balance competing professional and personal obligations. Increasingly, caregivers are younger people and from diverse backgrounds, creating new hurdles for government and social welfare organizations trying to reach these individuals.  
A new journalism collaborative is hosting a Caregivers on the Front Lines Public Forum at 7 p.m. on Thursday, Sept. 9, to explore solutions for the complex issues that caregivers encounter. This free live, virtual event will be hosted by Pulitzer Prize-winning journalist Stephen Henderson and carried on Detroit Public Television’s digital platforms, including <a href="https://www.facebook.com/events/195849815941653">Facebook</a>. 
The <a href="https://nymisojo.com/">New York &amp; Michigan Solutions Journalism Collaborative</a> was formed this year by <a href="https://nymisojo.com/about">more than 30 media, academic and community organizations</a>, initially to focus on the lives and challenges of caregivers.

Panels will discuss potential solutions on a range of crucial issues including the growing shortage of paid direct care workers, the urgent need for respite care, support for communities in which many caregivers and care recipients do not speak English, and training for family members who find themselves thrust into but unprepared for the role of caregiver, among other topics. 
Henderson is the host of “American Black Journal” on DPTV as well as Detroit Today, the morning show on 101.9 WDET, Detroit’s NPR Station. 
The event is being produced in collaboration with <a href="https://stridesforseniors.org/">Strides for Seniors</a>, an annual month-long series of activities celebrating Detroit’s neighborhood Senior Centers, organized by Detroit Area Agency on Aging, Detroit’s Parks &amp; Recreation and St. Patrick Senior Center. 
“Our collaborative’s journalists have produced insightful, solutions-based coverage, which has brought a new and needed focus to issues of caregiving in our communities,” said Karen Magnuson, project director. “Their reporting has given a human face to the problems faced by caregivers, while at the same time identifying available programs and resources to help them provide better care.”
“The public forum will help us deliver this important content to a larger audience and give caregivers of older adults a voice and an opportunity to inform us about the issues that matter most to them.”

Typically, Farzana Noor, a nurse practitioner at the Children’s Clinic of Michigan in Hamtramck is accustomed to bandaging up cuts and scrapes, treating colds, and soothing fussing children getting their first shots.
That all changed last year when the pandemic raged through the Bangladeshi community in Hamtramck. When nearby healthcare providers abruptly ceased operations amid the crisis, Noor began seeing an influx of older Bangladeshi women patients who often serve as the primary caregivers spanning generations in their households enter her office.

Noor, who is Bangladeshi American, found herself in familiar territory as many of the older patients were accompanied by their children to help translate care plans from English to Sylheti.
“When I have (an older) patient, there are certain things that even though they kind of understand what I’m telling them, if the child understands it, they can help reiterate it when they get home,” she says.
“It’s almost like a form of accountability.”
Language is a longstanding obstacle in diaspora communities, making everything from understanding utility bills to letters from teachers to public assistance correspondence difficult to navigate.
In traditional Bangladeshi families, the lines between caretaker and those in their care are blurred as children often translate communications to parents, grandparents, aunts, and uncles. Likewise, caretakers may be well into their 50s and are charged with looking after small children and aging parents alike.
Rebeka Islam, executive director of Asian and Pacific Islander American Vote-Michigan a nonpartisan civic engagement organization, says the group developed and distributed a sharable <a href="https://drive.google.com/drive/folders/1KmaFpW-NFoKy8_D0JrNKpUsNb15MFkSk">Google drive </a>featuring wide-reaching information to address the dilemma of language access during the pandemic.
Proper hand-washing, how to social distance, travel safety precautions, and how to properly cover one’s mouth, were just a few tips that English-speakers may take for granted published in the Google drive in dozens of languages.
“As we know… a number of our Asian American and Pacific Islander communities fall under the radar and don’t have access to the same resources that other communities do,” she says.
“Oftentimes, this means that individuals whose first language is not English won’t be able to get critical information because that information is not translated into the languages they speak,” Islam added
That’s certainly the case for the Bangladeshi community in Michigan, Islam says. Most recently, APIA Vote-Michigan undertook a similar effort by collaborating with others to translate voting materials for the 2020 election.
Metro Detroit is home to the third-largest Bangladeshi-American community in the United States. According to data analysis <a href="https://www.pewresearch.org/social-trends/chart/top-10-u-s-metropolitan-areas-by-bangladeshi-population/">published in 2017 by the Pew Research Center</a>, Detroit’s Bangladeshi population was 10,000. APIA-Michigan’s Islam says the population size is not well documented but estimates the number in Michigan at anywhere <a href="https://yemeniamerican.com/en/bengali-ballots-in-hamtramck-whats-at-stake-for-bengali-voters-during-the-election/">between 30,000-100,000</a>. The community only trails in size behind New York City and Washington, D.C. As the coronavirus ravaged throughout Metro Detroit in 2020, the region’s Bangladeshi community was hit especially hard.
Many Bangladeshi households are made up of several generations of family members making social distancing difficult. These households are more likely than the general population to live in poverty (19 percent of U.S. Bangladeshis versus 13 percent of all Americans, <a href="https://www.pewresearch.org/social-trends/fact-sheet/asian-americans-bangladeshis-in-the-u-s/">according to the Pew Research Center</a>). What’s more, not all Bangladeshis speak the same dialect of Bangla. In Metro Detroit a majority of Bangladeshi-Americans who speak Bangla use the Sylheti dialect.
Noor says Shuddho Bangla — the national dialect of Bangla in Bangladesh — is the most often used in translated materials, spelling a problem for Bengalis in Metro Detroit.
With some of these obstacles in mind, it’s clear to Noor and other healthcare providers that a more holistic approach is needed.
In New York, social services organizations that serve the South Asian population are finding other ways to reach the community and provide an added essential layer of care during the pandemic.
Shyda Rashid, an anti-violence program manager of Sakhi for South Asian Women in New York, an advocacy organization that connects people to resources, says many of the people the organization contacted needed language assistance when they were hospitalized after contracting COVID-19.
“(About) 80 percent of our clients have language barriers, you know they prefer to speak in their native language,” she says.
Rashid says the city of New York’s Language Access Plan helps to fill some of the gaps. Bangla is among 10 languages available for free interpreter services through the program.
“We always encourage our clients to say to every service provider that I need an interpreter because interpreter or translation service is a right of New York City.”
Still, the language line has its limitations. Callers can spend a long time on hold until they connect with a service provider, interpreters do not always show up on time, or incorrectly interpret the information or by inserting their own opinions or voice, and some do not speak the same dialect. Rashid also said that gender can also play a role in the quality of the care that callers receive.
What the organization has found impactful is going beyond translating by offering other services that help to build trust within the community.

Throughout much of the pandemic in 2020, the Sakhi group conducted about 1,500 wellness calls and needs assessments in the South Asian community, increased the organization’s caseload from 400 in 2019 to 450 in 2020, and provided cash assistance ranging from $300 to $1,500 to 50 to 60 clients. She says Sakhi also helped pay for groceries and medication for those who are undocumented from the Sakhi Solidarity Fund.
Sakhi’s <a href="https://www.sakhi.org/covid19-updates-resources/">website</a> includes an extensive list of resources available during the pandemic including mental health, food pantry locations, mutual aid, bill assistance, and infographics on how to develop a safety plan. The group’s pro bono legal partners also provide legal support on family law and immigration, including workshops and clinics. Sakhi a weekly newsletter for 739 clients. Much of the information published by the group is available in nine languages, including Bengali, Hindi, and Urdu.
In one instance, Rashid says, a Bangladeshi mother of a newborn and 3-year-old who was hospitalized due to COVID-19, contacted Sakhi for assistance. They were able to find childcare for the kids by splitting the cost with Project NewYorks, a nonprofit in Queens  that provides free babysitting services as the family recovered from COVID-19.
Rashid says the pandemic brought up issues beyond the virus. Anxiety and depression spiked among clients — many of whom are survivors of intimate partner violence — who lacked a safe space once they were stuck home in one-bedroom apartments.
<a href="https://www.laalnyc.org/mission">Laal NYC</a> (laal means red in Bangla, symbolic of Bangladeshi women empowerment) is another community-based service organization in New York serving Bangladeshis in the Norwood neighborhood in the Bronx.
Ishrat Aishee, a strategic development coordinator of Laal, said that last year, Laal organizers had intended on making calls to remind community members to fill out the U.S. Census, but they quickly pivoted to include wellness and needs assessments during those calls.
What organizers found while making phone calls was that many people — about 40 to 50 percent of the population the group serves are undocumented — were cut off from pandemic relief, food, and other city services.
“We ended up partnering with Bengali Mental Health Initiative (another NYC-based nonprofit) and disseminated pamphlets that were culturally conscious of certain language and phrasing to really identify what symptoms are and (what they) can lead to.”
Aishee says people fluent in Sylheti or Shuddo were also hired to communicate directly with the community to find out what their needs were.
Laal hired a substantial number of Sylheti-speakers to provide classes and other programming in the Norwood neighborhood.
“They don’t have the resources at home or people to seek help from, in that sense, and so this is where all of our conversations (with) our outreach team” come in to fill that gap, Aishee says.
While this holistic infrastructure for support in Metro Detroit’s Bangladeshi has a ways to go before matching the impact of organizations in New York, Noor and others have taken some steps to get there.
Noor partnered with Kroger Pharmacy in March and April to provide pop-up vaccine centers, serving more than 2,000 people. In May, Children’s Clinic of Michigan became a vaccination site.
“On average we vaccinate 150 to 200 people on Saturday and Sunday, and 75 to 100 people Monday through Thursday,” she says.
Locally, a group of researchers from Eastern Michigan University in Ypsilanti is working to identify the healthcare needs of the Bangladeshi community so they can take that next, more holistic approach.
EMU’s Center for Health Disparities Innovations and Studies developed a survey to learn the information needs of people in underserved Asian American communities in Michigan — including the Bangladeshi community. The center is one of nearly 40 organizations throughout the United States that have received funding from the U.S. Centers for Disease Control and Prevention’s Racial Ethnic Approaches to Community Health (REACH) program. The EMU group recently received additional funding to educate Hamtramck’s Bangladeshi population about flu and COVID-19 vaccines.
Sarah Lally, a nurse educator, and coordinator for the center who served as a project manager for the EMU initiative says the group of researchers worked with the CDC to identify social determinants — such as socioeconomic status and sparse access to healthcare providers — in specific ZIP codes to find out which populations need the most assistance. The information was used to determine where more education was needed.
The team also leaned on trusted members of the community — physicians, elected officials, as well as with researchers at social service agencies like J&amp;E Community Service and Detroit Friendship House — to gather and inform them on concerns they heard from residents. They held two listening sessions and trained people to help residents learn how to speak about COVID-19 myths and facts, Lally says.
“We can sit back and guess what people want to know, but why don’t we ask people what they want to know, you know, why don’t we find out what people’s concerns are,” Lally says. “We’re asking people to be the boots on the ground and messengers.”
The result, researchers were able to identify the information needs of the community, vetted that information, and printed materials in Bangla using professional translation services. REACH partnered with Hamtramck Public Schools to share information via the school messenger system Class Dojo.
The team also provided six pop-up COVID-19 vaccine clinics in Metro Detroit, with Bengali and Bangla speaking language interpreters, an effort that vaccinated  500 people.
“We are well aware of the inequities that exist, and personally I’m very excited to get to a point where we can start getting some people vaccinated so that then, word of mouth can spread, you know because that’s one of the biggest things that people have told me is the best way that we get information to each other is by talking to one another,” Lally says.
Lally says the goal of this initiative is to drive sustainable policy change for communities by working with city, state, and federal representatives.
That information could prove to be vital to informing other healthcare providers in meeting Metro Detroit’s Bengali population where they’re at.
Noor says the Bangladeshi community is accustomed to taking care of health problems rather than seeking preventative measures, such as maintaining good health and lifestyle. She says this is directly correlated to low literacy levels.
“This community definitely needs a lot more community resources outside of just within the doctor’s office,” she says.
She says, because many Bangladeshis moved to Michigan in the late 1990s and 2000s, they are still the first generation. She says many children of immigrants are the first ones who went away for college and have access to resources to give back to the community.
“It’s up to our generation to kind of step up and help (by) coming together to help provide some of these services and make healthcare more available,” Noor says.

As more and more Americans take on the responsibility of caring for an aging family member, not enough caregivers are taking time care for themselves. This can lead to stress and health issues.
Respite care provides short-term relief for caregivers by allowing them to step away for a break and personal time, while trained professionals take care of their loved one either in the home or at an outside facility. However, many family caregivers don’t know about respite care or don&#8217;t know how to find these services, a price they can afford.
In this report for the New York &amp; Michigan Solutions Journalism Collaborative, One Detroit and Urban Aging News take a look at an adult day program that is helping caregivers get much-needed respite care. The DayBreak program at the Hannan Center had only been open for six weeks last year when the pandemic forced it to suspend its operations. Now, it has opened again, providing services, activities and socialization for older adults with dementia as well as a much needed break for their regular caregivers. The report talks to the people who run DayBreak and a very grateful caregiver, as well as providing helpful advice on selecting a respite care provider.
